Directions for Completing the Registration Form

1. Please download the registration form first (desktop works best)
2. Type your answers directly into the blue boxes, using the tab key for best results
3. Save the form
4. Attach the form back to langdonlearningcentre@gmail.com
5. An administrator will respond via email if a spot is available. Once the administrator confirms, we will instruct you as to when the $75 non-refundable registration fee is due.

 PLEASE DO NOT SEND IN PAYMENT UNTIL YOUR REGISTRATION HAS BEEN CONFIRMED 

• Only SARAH TOMPSON SCHOOL students must fill out the TRANSPORTATION WAIVER FORM.
